To help make your first visit as efficient as possible, please complete your forms before your appointment whenever possible. 
 
Please print all forms single-sided. Complete forms by hand (do not use adobe forms or google docs) and bring them with you to your appointment. Please do NOT scan and email completed forms to our office.
